Patient Privacy Notice

Range Health & Performance respects your privacy and is committed to protecting your personal and health information.

We collect information that is reasonably necessary to provide safe and appropriate care, manage appointments and payments, communicate with you, and meet our professional and legal obligations. This may include your contact details, medical history, health information, treatment records, imaging, referrals and payment information.

Your information will only be accessed by authorised persons and will not be shared with third parties without your consent, except where required or permitted by law, or where necessary to provide your care.

You may request access to, or correction of, the personal information we hold about you. Privacy questions, requests or complaints can be directed to Range Health & Performance at admin@rangeperformance.com.au.

Terms and Conditions

By booking an appointment with Range Health & Performance, you acknowledge and agree to the following terms and conditions.

1. Appointments and care

Appointments are subject to practitioner availability. Booking an appointment does not guarantee a particular treatment or outcome.

Your practitioner will assess whether the proposed care is appropriate for you. The nature, expected benefits, material risks and available alternatives will be discussed where relevant, and your informed consent will be obtained before care is provided. You may ask questions or withdraw your consent at any time.

2. Information you provide

You agree to provide information that is accurate, complete and relevant to your care, including your medical history, current symptoms, medications and any changes to your health.

Please tell your practitioner if you are pregnant, may be pregnant, have recently undergone surgery, or have experienced a significant change in your health.

3. Fees and payment

Appointment fees will be communicated when booking and are payable at the time of your appointment unless otherwise arranged.

Private health insurance rebates depend on your individual policy and eligibility. Range Health & Performance cannot guarantee that your insurer will provide a rebate. You are responsible for any remaining appointment fees.

4. Cancellation policy

Please provide at least 24 hours’ notice if you need to cancel or reschedule your appointment.

Same-day cancellations and failure to attend an appointment will incur the full appointment fee. This policy helps us manage appointment availability and offer cancelled times to other patients.

Exceptional circumstances may be considered at the discretion of Range Health & Performance.

5. Late attendance

If you arrive late, your appointment may need to be shortened to avoid affecting other patients. The full appointment fee may still apply. If there is insufficient time to provide appropriate care, the appointment may need to be rescheduled.

6. Privacy and communication

Range Health & Performance collects, stores, uses and discloses personal and health information in accordance with applicable Australian privacy laws and professional confidentiality obligations.

Your information may be used to provide and coordinate your care, manage appointments and payments, send appointment-related communications, and meet legal or professional obligations.

7. Referrals and third parties

With your consent, relevant information may be shared with other healthcare practitioners, referring providers, insurers or members of your care team where reasonably necessary to coordinate your care.

Information may also be disclosed without consent where required or permitted by law.

8. Emergencies

Range Health & Performance does not provide emergency medical services. If you are experiencing a medical emergency, call 000 or attend your nearest emergency department.

Online information and electronic communication with the clinic should not be relied upon for urgent medical advice.

9. Australian Consumer Law

Nothing in these terms excludes, restricts or modifies any right, guarantee or remedy that cannot lawfully be excluded under the Australian Consumer Law or other applicable legislation.

10. Changes to these terms

These terms and conditions may be updated from time to time to reflect changes to our services, fees, policies or legal obligations.

The current version and its effective date will be made available through our booking platform or website. Where a change materially affects patients, we will take reasonable steps to provide notice before it takes effect.

Changes will generally apply to appointments booked after the updated terms take effect. They will not retrospectively alter the terms of an existing booking unless required by law, necessary for patient safety, or agreed with you.
